For the curious consumer: Batch-level testing is not common practice among manufacturers. A great example of the lack of testing in this space is the massive Zanac (randitidine) recall in 2019 and 2020. Manufacturers recalled ranitidine (both brand and generic) after a cancer-causing chemical called N-Nitrosodimethylamine (NDMA) was discovered in every medicine lot.
Cabinet Cough Relief, a FDA-approved generic alternative to Robitussin® Long-Acting CoughGels, is a popular over-the-counter (OTC) cough medicine used to temporarily relieve cough caused by minor throat and lung irritation that may present with a cold. The active ingredient, dextromethorphan hydrobromide, belongs to a group of medications called antitussives (or cough suppressants). Dextromethorphan is able to cross into the cough center of the brain to suppress the cough reflex.
Good to know: This ingredient only works to suppress cough and does not work to relieve any other pains associated with being sick. Because of this, dextromethorphan is commonly combined with pain relievers and other medicines for overall common cold or flu relief. A great example is Cabinet Daytime Severe Cold & Flu, which contains ingredients that will help relieve aches and pains, sore throat, congestion, mucus, and cough.
Adults and children over the age of 12 can take two (2) capsules every six to eight hours as needed for cough. Do not take more than eight (8) capsules of Cabinet Cough Relief in 24 hours. The effects of dextromethorphan should start to kick in after 30 to 60 minutes.
This product is not approved for pediatric use and is not recommended for children under the age of 12, unless directed by a doctor. Cabinet Cough Relief should be stored away from young children. In the case of an overdose, contact a Poison Control Center (1-800-222-1222) or seek medical attention immediately.
In addition to its desired effects, dextromethorphan may cause some less desirable side effects. Everyone is different, so some people may experience no side effects at all, others may experience mild side effects, and a small number of people may experience rare or more serious adverse effects.
Some side effects of dextromethorphan include: lightheadedness, drowsiness, dizziness, restlessness, nervousness, nausea or vomiting, and stomach pain. Contact your doctor immediately if you have a bad rash after taking a tablet. Some patients may experiece side effects that are not on this list. Check in with your health care provider if you have questions or experience concerning side effects.
Always check with your doctor or pharmacist prior to taking Cabinet Cough Relief if you have ever had an allergic reaction to any of the ingredients the product contains.
It is important to note that Cabinet Cough Relief is NOT for everyone. Do not use dextromethorphan if you are currently taking a monoamine oxidase inhibitor (MAOI) or have taken a MAOI in the past two weeks. If you were on a MAO inhibitor before, make sure to wait at least two weeks before taking Cabinet Cough Relief. MAOIs (i.e. isocarboxazid, phenelzine, quinidine, rasagiline, selegiline, tranylcypromine) are a group of prescription drugs used to treat depression, psychiatric or emotional conditions, or Parkinson's disease. When dextromethorphan is taken with a MAOI, the body's production of serotonin may be severely increased and cause adverse effects. Elevated serotonin levels can alter body temperature, blood pressure, and behavior. These changes can lead to a life-threatening medical condition called Serotonin Syndrome. If you are unsure if your prescription contains a MAOI, make sure to speak to your doctor or pharmacist before taking this medicine.
Cabinet Cough Relief is generally considered to be one of the safer cold medicines to use while pregnant, and it is not known to affect breast milk in breastfeeding mothers. However, not all people and pregnancies are the same! Make sure to consult with a doctor or healthcare provider before use.
It is recommended to limit or avoid alcohol intake while you are taking Cabinet Cough Relief. The use of dextromethorphan with alcohol can increase certain side effects including dizziness, drowsiness, and difficulty concentrating. This combination may also cause impaired judgment and thinking, similar to being intoxicated. It is important to be aware of the maximum daily dose of Cabinet Cough Relief and to avoid activities that require mental focus such as driving or operating hazardous machinery until you know how the medicine affects you.
Please speak with your doctor prior to using Cabinet Cough Relief if you have:
A cough that presents with significant phlegm (mucus)
A lingering (or chronic) cough that is a result of smoking, chronic bronchitis, asthma, or emphysema
Cabinet Cough Relief provides temporary relief of cough. You should stop using this product and seek medical advice from a health care professional if your cough:
Lasts for more than seven days
Returns after a while
Presents with other symptoms such as fever, rash, or headache
These presentations could be signs of a more serious condition or infection.
