For the curious consumer: Batch-level testing is not common practice among manufacturers. A great example of the lack of testing in this space is the massive Zanac (randitidine) recall in 2019 and 2020. Manufacturers recalled ranitidine (both brand and generic) after a cancer-causing chemical called N-Nitrosodimethylamine (NDMA) was discovered in every medicine lot.
Cabinet Stomach Relief, a generic alternative to Pepto-Bismol® Softgels, is a popular over-the-counter (OTC) medicine used for nausea, heartburn, indigestion, upset stomach, and diarrhea. The active ingredient, bismuth subsalicylate, belongs to a drug class called salicylates. This ingredient is thought to work in the gastrointestinal tract by absorbing water and toxins in the large intestine, decreasing the amount fluid and electrolytes that enter the bowel, reducing inflammation in the intestines, and killing harmful organisms that cause diarrhea.
Cabinet Stomach Relief is a medicine used to relieve diarrhea and travelers' diarrhea. This product can also help with other gastrointestinal symptoms caused by over indulgence in food and drink including: heartburn, abdominal pain, indigestion, nausea, gas, belching, and fullness.
Fun fact! Bismuth subsalicylate is often also used as part of a combination treatment for H. pylori infections.
Children and teenagers who have or are recovering from chicken pox or flu-like symptoms should avoid this product. If your child or teen falls into this boat and used a salicylate-containing product, seek medical help immediately if you notice behavior changes along with nasuea and vomiting. These could be early symptoms of Reye's syndrome, which is a rare but serious illness.
Cabinet Stomach Relief's active ingredient is bismuth subsalicylate, so if you are allergic to salicylates (such as aspirin) or are using other salicylate products, do not take this medicine. Avoid taking this product if you have an ulcer, a bleeding problem, bloody stool, or black stool. Ask your doctor before using Cabinet Stomach Relief if you currently have a fever or have mucus in the stool. Bismuth subsalicylate does have a few drug interactions, so check in with a doctor or pharmacist if you are taking any drug for blood thinning (i.e. anticoagulants like warfarin or daily aspirin), diabetes, gout, or arthritis.
Stop using Cabinet Stomach Relief and speak to your doctor if you:
Experience worsening symptoms or symptoms that last for more than two days
Experience ringing in the ears or hearing loss
Notice changes in behavior that occurs with nausea and vomiting
These may be signs of something more serious.
Although rare, seek medical attention immediately if you develop a serious allergic reaction to this product.
Adults and children 12 years and over may take two (2) softgels (1 dose) every 30 minutes OR four (4) softgels (2 doses) every hour as needed for antidiarrheal purposes. Similarly, adults and children 12 years and over may take two (2) softgels every 30 minutes as needed for symptoms overindulgence. Do not take more than eight doses (or 16 softgels) in a 24 hour period. Each dose should be swallowed whole with a glass of water.
If you are taking this product for diarrhea, make sure to drink plenty of fluids throughout the day to prevent dehydration. Cabinet Stomach Relief can be used until diarrhea stops, but cannot be used for more than two days.
This product is not recommended for children under the age of 12, unless directed by a doctor. Cabinet Stomach Relief should be stored away from young children.
In the case of an overdose, contact a Poison Control Center (1-800-222-1222) or seek medical attention immediately.
The FDA recommends against the use of bismuth subsalicylate-containing products during pregnancy. Data suggests this medicine can increase risk of bleeding or birth defects in all trimesters of pregnancy. Similarly, research also suggests that bismuth subsalicylate is not safe to take while breastfeeding.
It is recommended to seek medical advice from a health care professional if you are looking for a safe treatment option for symptoms of diarrhea, upset stomach, heartburn, indigestion, or nausea.
In addition to its desired effects, Cabinet Stomach Relief may cause some less desirable side effects. Everyone is different, so some people may experience no side effects at all, others may experience mild side effects, and a small number of people may experience rare or more serious adverse effects.
Some side effects of bismuth subsalicylate include: constipation and dark colored stools. Note that this product can commonly cause temporary blackening of your stool or tongue. This is a normal side effect of using the product and is no cause for concern. More serious side effects of bismuth subsalicylate include: ringing in the ears, loss of hearing, and worsening stomach symptoms. Some patients may experience side effects that are not on this list. Check in with your health care provider if you have questions or experience concerning side effects.
Always check with your doctor or pharmacist prior to taking Cabinet Stomach Relief if you have ever had an allergic reaction to salicylate or any other ingredients this product contains.
